Registration form is enclosed with this FINAL ANNOUNCEMENT. Completed form must be returned with the registration fee payable to the Diabetes Malaysia by mail or fax. Registration fee covers admission to all Conference sessions, exhibition, lunch and tea / coffee breaks. The fees are as follows:

Registration and Cancellation Policy

Please register early to enjoy “Early Bird” discount. Payment should be made payable to: DIABETES MALAYSIA. Confirmation of registration will be sent via email /fax/mail to all participants upon receipt of full payment. Please note that all related bank or financial charges are to be borne by the participants and are not to be deducted from the fees payable to the Diabetes Malaysia.

Any cancellation of registration must be made in writing to the Conference Secretariat. Refunds will only be made after the Conference.

Cancellation received on or before 30th June 2017 : 100% refund

Cancellation received from 1st July to 15th July 2017 : 50% refund

Cancellation received on or after 15th July 2017 : No refund for whatever reasons

Diabetes Malaysia [DM]

Diabetes Malaysia (DM) was formed in 1981 and registered in 1983, under the Societies Act 1966, as a non-profit, non-governmental organization to assist people with diabetes to lead healthy and productive lives. DM has 14 State Branches and 38 District Branches with 12395 members in 2016.

Some Objectives of DM are:

  1. To provide services to all people with diabetes in Malaysia and others interested in diabetes.

  1. To raise funds to provide assistance (such as monitoring devices) for eligible poor and needy children and adults to better control their diabetes.

  1. To organize and participate in conferences, seminars, forums, camps, courses, exhibitions, and workshops on diabetes and related fields.

  1. To compile, print, publish and distribute booklets, brochures, pamphlets, newsletters and other reading materials on diabetes and related matters.

  1. To buy and sell diabetic products, equipment and monitoring devices for the use of members and people with diabetes at reasonable prices.
